The Schizophrenia Society of Ontario (SSO) is committed to making a positive difference in the lives of individuals, families and communities affected by schizophrenia and psychotic illnesses. To ensure that the mental health system is truly responsive to the needs of individuals and families impacted by these conditions, SSO is creating our first-ever Report on Schizophrenia and Psychosis in Ontario.

This is where we need your help! Input from individuals living with schizophrenia, psychotic illnesses, and experience of psychosis is not only invaluable, but will serve as the foundation for our report.

To assist you in sharing your feedback and ensure that your suggestions and concerns are heard by political decision-makers, we developed a brief, anonymous survey. The survey will focus on your experience with programs and services currently available through the mainstream mental health system. You will also have opportunities to share your feedback on what other formal or informal supports, if any, you found helpful.

The survey takes about 20 minutes to complete. All information gathered through this survey will be kept confidential.
