Program Description: 

This learning collaborative is an opportunity for health center teams to work together using a social justice framework to build inclusive and affirming health care environments for all bodies, particularly for delivering diabetes related care for queer patients. Each session will have both a didactic component and group discussion. Participants will conclude the learning collaborative by developing an action plan to identify and address organizational changes needed to improve care for queer patients with diabetes.  

As a result of participating in this program I will:
  • Learn well-researched methods of delivering high-quality, culturally affirming care for queer people, and discover effective methods and tools to improve health outcomes in diabetic patients.
  • Understand how to use a social justice framework to build inclusive and affirming health care settings for all bodies, particularly people from the queer community.
  • Build relationships with other organizations to facilitate peer learning experiences and discover resources for training implementation.
  • Collaborate with peers and expert facilitators to identify and plan organizational changes needed to improve care for queer patients living with diabetes.

Participation guidelines:

To participate in the learning collaborative you must:
  • Have team members who can attend all 4 sessions
  • Participate actively
  • Be a Federally Qualified Health Center (FQHC) or look-alike.

Program Structure:
Health centers will participate in 4 Tuesday sessions in June of 2023 at 1:00pm - 2:30pm EST/ 10:00am-11:30am PST
on the following dates:
  • June 6, 2023
  • June 13, 2023
  • June 20, 2023
  • June 27, 2023
