A $25.00 donated item is REQUIRED to be raffled as door prizes. “Golden Sticker” will be inserted in the guidebook and seniors will be able to pick up their prize at the day of the event. Stickers are printed and inserted in guidebook based on the number of registered vendors, so please do not forget to bring an item. Items can be dropped off any day before the event at the office, Zion Township 1015 27th Street, Zion. Please find a Zion Township staff at set up the day of event to drop off your donated item.
Agency information will be listed in the guidebook, along with a short description of services offered (as completed by you on this form) that will be distributed at the event. Once your reservation has been confirmed, we will send you a confirmation with detailed instructions for the day of the event.
