Hello,

This form is to support participant application onto the DFI Community Participation Training Network (CPTN) Training Courses being delivered throughout Ireland this Autumn. Full information on CPTN, course descriptors and and what we are trying to support is available online through our website www.disability-federation.ie.

Each of the areas the course will be delivered in is being supported locally, with key contacts listed below for you to link in with you have any queries:

Dublin - Regarding the course in Accessibility and  Universal Design - Cathy McGrath 086 3847440 / cathymcgrath@disability-federation.ie

Galway - Sabrina Cummins 087 930 34 68 / Sabrina@gcp.ie

Mayo – Deirdre Fahey 094 903 4980 / dfahey@mayocil.ie

You will now be asked to complete a few short questions as part of the application process. Please note we will only deliver a course if we have enough people! Once your application has been received, your local contact for the course will give you a call to talk it through on the phone, or meet up to give you a good sense of what its all about, and answer any concerns or questions you might have in advance. It will also be a great chance for you to meet the people who locally, are active in the community, and who may be of support even after the course.

Thank you for your time, its valuable!

Kind regards

Cathy McGrath, DFI
