This is your weekly check-in to keep you and the people you work with safe during the COVID-19 pandemic. Please fill this in every Wednesday so that we have all the information we need in order to book you for the following week. 

It takes about 3 minutes to do the 1st time and less than a minute to do subsequently, if nothing has changed in relation to COVID-19.
