Acknowledgement

We remember all those we have lost to suicide and acknowledge all people who have experienced the loss of a loved one by suicide.

OPPORTUNITY TO HELP INFORM THE WESTERN AUSTRALIAN SUICIDE PREVENTION ACTION PLAN 2021-2025.

We would like to hear your thoughts and ideas about suicide prevention activity in your community.

The Mental Health Commission (MHC) is developing a new suicide prevention strategy for Western Australia. The new strategy will build on the work of Suicide Prevention 2020: Together we can save lives and will provide a guide for Government, non-government and the community to reduce the incidence of suicide and self-harm across our state.

This survey will complement a series of face to face consultations currently being undertaken in the community across Western Australia from June 2019 to August 2019.

The Survey

This survey will likely take approximately 15 minutes to complete.

It is completely anonymous and all questions are optional. It will ask for your thoughts about the type of activities you believe will help to address suicide in your community.

Some of the topics covered in this survey may be challenging. If your participation raises any concerns for you, or if you or someone you know would like to talk to an experienced counsellor, please call one of the numbers listed below.
