The North Bay Regional Health Centre is looking to the community for input to help determine our Health Centre’s priorities and build a strategic plan for the next three years. The strategic planning exercise not only determines where we are going, and what is needed to make progress, but also how we will know if we are successful.

PURPOSE

The information you provide in the survey is important because it will help answer the question: What are the hospital's current strengths and areas for improvement and what opportunities and challenges exist externally that we need to consider when developing the 2017-19 NBRHC strategic plan? 
 
The survey is voluntary and will be kept anonymous; however, there is an option to identify yourself if you wish to receive additional information.

It will take ~10 min. to complete and the survey closes August 30, 2016.
