Thank you for taking the time to complete this exit survey. The purpose of this survey is to gather data on why people choose to leave employment with the State of Colorado or the reasons why they choose to leave one agency for another.

Your candid comments are greatly appreciated. Data may be shared with Human Resources Directors, but will not be linked to you or the date the survey was taken, nor be placed in your personnel file. To assure this, we ask you not to provide your name and contact information. In addition, completion of this survey is not part of any grievance process, nor does is constitute official notice of violations of federal or state law.
