Engagement Survey

Southern NSW Local Health District covers a large area of the state, from Goulburn and Crookwell to the Victorian border and surrounds the ACT on three sides. We are home to four large Aboriginal nations – the Gundungurra, Ngunnawal, Ngarigo and Yuins.

The Southern NSW Local Health District community consultation survey forms part of the engagement for the development of the SNSWLHD Health Care Services Plan 2020-2030. This Plan identifies the priorities for the health system to concentrate on, and direct resources towards over the next 10 years.

This survey will ask about you, so we can identify that we have captured important groups within your community. We will then ask you about the services that are important for you and your community.

This survey will take around 10 minutes to complete.
