Welcome to the Aged Care Facility Survey for General Practitioners.  Sydney North Primary Health Network is committed to working with you to improve access, availability and quality of care for people living in residential aged care facilities in the Northern Sydney PHN region. This survey comprises 6 questions and will take around 5 minutes to complete. The survey will assist SNPHN to map GP attendance rates at these facilities and identify any issues you may be experiencing.
