Introduction

As COVID-19 restrictions are being lifted across the province, we are planning to gradually reopen and expand our in-person mental health services. The health, safety and well-being of our clients, employees and the public remains our top priority as we begin the process of returning to normal operation.

Your input as a parent/caregiver is essential in informing the development of our pandemic recovery plan and post-pandemic service delivery. We would like to invite you to complete this brief survey to help us better understand your needs and concerns related to accessing in person mental health services either at Crossroads Children’s Mental Health Centre (Crossroads) or at home.

Taking part in this survey is optional and voluntary. You may choose to stop answering the questions at any time.

This survey is anonymous. The results will highlight the overall responses for the whole community.

Thinking about the gradual reopening of in-person mental health services, please answer the following questions:
Crossroads is committed to keeping clients, employees and the public safe. Following the latest public health guidelines, Crossroads will:

·         Enforce mandatory COVID-19 screening of all staff and clients prior to each in-person session

·         Limit the number of clients allowed on-site at any given time

·         Arrange the waiting area to ensure proper physical distancing

·         Maintain physical distancing

·         Wear personal protective equipment where physical distancing cannot be ensured

·         Ask clients to wear masks during in-person sessions
