Before you start:
Please have all the information about your nomination prepared in advance as you cannot come back and finish at another time. Please submit only one application per nominee. Multiple applications for the same individual / team will not be assessed or evaluated.

Nominations will be assessed and evaluated against the agreed criteria, which is available on our website midulstercouncil.org/awards.


The Mid Ulster Sports Awards aims to recognise those people who have excelled in the world of sport and recognise the valuable contribution and time commitment of individuals and teams across the Mid Ulster District Council area.

Nominations are welcomed from sports clubs, physical activity groups, schools and members of the public across the nine award categories for teams and individuals, coaches, officials and volunteers.

It's easy to nominate, just complete the nomination form and submit before the closing date of 12 noon on Friday 17 October 2025.

The Assessment panel can only assess your nomination based on the information contained within this nomination form. Any prior knowledge will not be taken into consideration.

When scoring each nomination submission, the panel will refer to the detailed Mid Ulster Sports Awards Criteria which can be found online at midulstercouncil.org/awards.

The information you supply will be used solely for the purposes of administering the Mid Ulster District Council Sports Awards, including verifying eligibility, contacting nominees and nominators, and managing event logistics.

All data will be stored securely and retained only for as long as necessary to administer the Awards and meet any legal obligations.

Nominations will be assessed and evaluated against the stipulated criteria, which is available on our website midulstercouncil.org/awards. All nominees and nominators will be advised of the outcome of the initial assessment panel.

Shortlisted nominees will be invited to the the Mid Ulster District Sports Awards Ceremony on Thursday 20 November 2025 in the Burnavon Theatre Cookstown, at which the winners of each award category will be announced.

We will publish the information about the shortlisted nominees on our website at midulstercouncil.org/awards including their name and the award they have been nominated for. The decision of the judging panel is final.
