PERSONAL TRAINING AND EXERCISE THERAPY LIABILITY WAIVER AND INFORMED CONSENT Survey

The Client agrees to voluntarily participate in personal training, exercise therapy, fitness instruction, stretching, strength training, mobility work, cardiovascular conditioning, and related fitness activities provided by the Trainer.

The Client understands that participation in physical exercise and training activities involves inherent risks, including but not limited to:

Muscle soreness or strain
Sprains or tears
Joint injuries
Slips, falls, or accidents
Increased heart rate or blood pressure
Dizziness, fainting, or other physical stress
The Client acknowledges that these risks may occur despite reasonable precautions taken by the Trainer.

The Client affirms that they are physically capable of participating in exercise and training activities. The Client agrees to inform the Trainer of any medical conditions, injuries, limitations, medications, or health concerns that may affect their ability to exercise safely.

The Client understands that the Trainer is not a physician or licensed medical professional, unless otherwise stated, and that the training services provided do not constitute medical advice, diagnosis, or treatment.

The Client agrees to consult with a qualified healthcare professional before beginning any exercise program if they have concerns about their health.

I confirm that I am of legal age to give my consent.

The Client knowingly and voluntarily assumes full responsibility for any risks, injuries, damages, or losses that may occur as a result of participation in training or exercise therapy sessions.0

To the fullest extent permitted by law, the Client releases, waives, and discharges the Trainer, the Trainer’s employees, contractors, affiliates, and representatives from any and all claims, demands, damages, or causes of action arising out of or related to participation in training sessions, including claims resulting from negligence.

The Client grants permission for the Trainer to use photographs or videos taken during sessions for educational, promotional, or marketing purposes unless the Client opts out in writing.

The Client agrees to provide reasonable notice for cancellations or rescheduling of sessions. Specific policies may be outlined separately by the Trainer.

This Agreement shall be governed by the laws of the state of Pennsylvania; the business address of TrainingWithMandy, LLC; not the address of the client at the time of the training or thereafter.

By signing below, the Client acknowledges that they have read this Agreement, fully understand its terms, and voluntarily agree to its conditions.
