Instructions for using this Chart Abstraction Tool:
This Chart Abstraction Tool has been designed to help you determine your performance on key metrics related to multiple myeloma diagnosis and treatment for use with ABIM's self-directed Practice Improvement Module.
For each patient in your selection, answer all of the questions. When you’ve completed the questions for your patient, click ‘Done’ and follow the instructions to add another patient or to obtain your personal results.
The answers are recorded when you click ‘Next’ to move from page to page. If you are interrupted and need to leave the tool before the end, you will have a partial data set for the patient you were working on. If this happens, you will need to redo the questions for that patient and contact firstname.lastname@example.org to eliminate the partial data set from the results report.
