Thank you for taking the time to complete this questionnaire. The purpose of this survey is to help us find out how our suppliers operate, and how they complement the work that we do at Oxfam.
The questionnaire contains around 40 questions which focus on details about your organisation, as well as its products and services, policies and values. Before filling out and submitting your questionnaire, please read through the Oxfam Supplier Code of Conduct, as it is referred to in the survey.
