The purpose of this survey is to provide an easy way to update our photo directory. Please complete the survey to confirm/update your contact information (which will be pulled from our parish records). It is essential that you complete this survey if any of the following apply:
--Your contact information has changed

--You want to include your phone number and e-mail address in the directory
--You want to opt out from publishing any of your contact information in the directory
--You want to pay $20 to have your own digital photo included (instead of participating in the free Lifetouch photo session).
