General Questionnaire

In preparation for the 2024/2025 competition season, we need to know each dancer's plans for auditions and the upcoming season. If you are a parent filling this out, please complete it WITH your dancer.

We have multiple tracks to choose from, whether you want less of a time and financial commitment or if you're 100% all in and dance is your life. If you're the latter, the SIGNATURE Team Auditions are for you! Our SIGNATURE Team groupings encompass the Collective and Essentials Programs, with dancers selected via audition and requiring more of a commitment. If you are the former, and prefer to ease in to the competitive dance world, please look in to our STARS Program instead of SIGNATURE**.

**If you'd like to be considered for either the STARS or SIGNATURE competitive tracks, complete this questionnaire and attend the SIGNATURE auditions only, not the STARS audition. If only auditioning for STARS, there's no need to complete this survey.

Within the SIGNATURE Program, we offer six groups, referred to as "Headquarters":

- Baseline: Ages 6-8

- Reckless: Ages 9-11

- Custom Cut: Ages 11-12

- Mezzo: Ages 12-15

- Avant Garde: Ages 13+

- Elite: Ages 14+

IMPORTANT!
- The "Ballet-Based Styles" Audition determines placement for Jazz, Musical Theater, Contemporary AND Modern/Open pieces. If auditioning for these disciplines and you're offered a spot, you will be placed in the styles that are the most appropriate fit, not just for you, but for the groups as a whole.
- Tap and Hip Hop have their own separate auditions and placement is determined from those specific auditions.
