We would like to know more from you because your child has either received a transfer to a school outside Carman-Ainsworth Schools or applied for open enrollment in another school district.

In this survey we want to know about your experiences. We want to make sure you were treated well while in our schools and if not, how we could have done better. We want to learn if we need to make any changes to better serve our students and their families.

Please complete this questionnaire. Your responses will be anonymous and confidential. If you have more than one child leaving our school district, please feel welcome to complete a survey for each student.
