All fields are required to submit a customer service request - regardless of whether warranty or non-warranty.  Shorty after the completion of the request, you will receive an email with a CSR #.  Usually within 15 business days, you will receive another email or a phone call with additional information (warranty status, cost for non-warranty repairs, scheduling information).
