* 12. I am as a sponsor responsible to reach out to my local health department to conduct health inspection at my sites. If health inspections are not conducted at any of my sites by the end of my program, I understand that it is a finding that could lead to serious deficiency and I may have to return all the program reimbursements. I understand that as a school food authority I have to have 2 health inspections during the school year, if one is missing, I have to reach out to the health department and have the second inspection during the summer. As a school food authority, I understand that if the site is school site and had two health inspections during the school year, I don’t need a third health inspection during the summer program.  As a CACFP agency I understand if the site received health inspection during the CACFP operations that transitions into SFSP (or vice versa) I can accept the health inspection document from the program. I understand that FSMCs must have health certificates where they prepare the SFSP meals, they have regular health and meal quality inspections.
