2020-2021 SFSP/SSO Waiver Survey

Complete the following waiver survey to indicate which Summer Food Service Program (SFSP) and Seamless Summer Option (SSO) waivers your school division or organization will elect for school year 2020-2021. These waivers are in effect through December 31, 2020. Meals may be claimed under the SFSP or the SSO starting on September 1, 2020.
