Question Title

Please upload IRB Protocol form and any other supporting documents to the IRB Submission request system. Researchers should compile all required documentation listed below prior to submission. Once you have gathered the following documents in the checklist, please continue to the upload submission page. All documents indicated below may not apply to your submission. Complete IRB Protocol Form with all signatures. Date of Proposed Research field is REQUIRED.

A copy of the Grant Proposal if funding is involved (optional).

Informed Consent. Details here

Parental Consent (if subjects are under 18 years old).

A copy of any approved protocol for your research from another institution’s IRB (if applicable).

Any applicable Data Collection Instruments. (Surveys, questionnaires, interview guidelines, and recruitment materials). These must be uploaded as a single document.
