**Each family member is welcome to complete their own survey** Also, if you have already completed your own survey on paper and returned it to the Rectory, please do not duplicate it here.
On the following pages are a few questions - we ask for your honest opinions and recommendations that you wish to share. You may keep your responses completely anonymous, or share your name at the conclusion of the survey. The results of the survey will be reported at the Synod on March 20th. Please be assured that no names will ever be associated with the results and comments - this survey is completely confidential.
