County of Dufferin Community Services is conducting a Service Delivery Modernization project, which will improve services to provide an enhanced experience for people accessing any of our supports and programs in:

ONTARIO WORKS (financial assistance and employment services),
HOUSING SERVICES (subsidized and affordable housing, and programs including Homelessness Prevention, Home Ownership, Ontario Renovates, rent supplement and housing allowance),
CHILDREN'S SERVICES (Jean Hamlyn Day Care, Ontario Early Years Centre, Child Care Fee Subsidy, Early Literacy, Quality Assurance).

In this SURVEY, we are looking for feedback about your "first point of contact" with us (your experiences contacting us for the first time or accessing information about any of our services through a variety of sources). We would also like to have your ideas for how we can make improvements. Your participation in this process is important to help us understand your needs.

Please take a few minutes to answer all the questions (note that the survey may be double sided) and return to any one of our locations. We value your input!!

The deadline for survey completion and return is June 9, 2017.

If you would like to be entered into a draw for a $25 grocery gift card, please provide your name (first name is fine), and email or contact number at the end of the survey.
