Please take a few moments to share your experience with Resources for Exceptional Children and Youth (Resources) and your Service Coordinator*, reflecting on any interactions you experienced over the past 6-months when answering the questions below.

*“Service Coordinator” will be used as an inclusive term for any Resources staff member you have interacted with in the past 6 months including Access Coordinator, Coordinated Service Planner, Intensive Service Coordinator, FASD Coordinator, Transition Coordinator, Urgent Response Service Coordinator.
