The Kids Hope Alliance is seeking to better understand the needs of children, youth, and their families in Duval County, as well as the potential gaps that exist between these needs and what is currently being provided. 

A part of this work is to learn more about the organizations that serve children, youth, and their families in Duval County regardless of whether you are a KHA provider or not. There are two questionnaires:

This first survey is an inventory of basic information about your organization. These responses will be shared with the Kids Hope Alliance as they develop a full list of services available to children, youth, and their families in our city. This questionnaire should take about 10 minutes.

As soon as you are done, you will be directed to a second optional survey to share your perspectives and feedback as a provider of services to children, youth, and their families. This survey is anonymous and confidential, and your responses to it will only be shared in aggregate. The information you provide in this survey will not be tied to your organization. This questionnaire should take 5 minutes.
