Guidance for completing the DHL quarterly progress report
Welcome to the quarterly DHL progress report. This is your place to share all the great work you are doing for Head Start families and let the federal Office of Head Start know that registered dental hygienists are the perfect fit for helping Head Start families have better oral health. Here are a few things to remember to help you successfully complete this report:
* Since this report is in Survey Monkey, it must be completed in one sitting. There is no "save" function.
* Report only the activities you do in your role as a DHL. If you are unsure of those activities, refer to the current DHL At-A-Glance document on the DHL webpage. For example, the DHL role does not include providing direct preventive services, but it does include any activities you do to help facilitate the provision of those services. If you started providing direct care to Head Start kids through your work this quarter but you also were involved in connecting the program to your clinic, you can count that facilitation as a DHL activity.
