CONSENT TO TAKE PART IN RESEARCH AS A HUMAN SUBJECT

PROGRAM PARTICIPANT CONSENT FORM

What is the purpose of the study? We have come together to discuss concerns we have seen in our careers as well as on campus as it relates to various topics like mental health, substance use, sexual assault, and criminal behavior. We are hoping to gain insight into similar incidences of these topics in your library as well.  The overall goal is to gather your feedback in learning more about what is truly happening in our university and public libraries across the country to better plan for prevention and change.  

 

What will be done if you agree to take part in this research study?

By clicking next, you agree to take a survey about the overall professional climate that should take about 10-20 minutes to complete. Please take your time – and, by no means, is there any penalty for not participating. Further, you may stop at any time without penalty. In addition, we are asking if you would be interested in joining us for a possible future focus group session on the topics discussed on the survey at a later date.

 

What are the possible discomforts and risks?

We expect there to be minimal risks associated with your participation in this research. The potential risks include: boredom, anxiety, embarrassment, depression, or exacerbation of psychological condition. This survey may include the presentation and/or discussion of material which some participants may consider sensitive or offensive, threatening or degrading. 
We do ask for the location and name of the library where you are currently working. Additionally, there is an OPTIONAL secondary survey collecting contact information (name and email) if you would like to participate in future surveys or focus groups. If not, simply skip that optional secondary survey. 

 

What are the possible benefits to taking part in this research?

This research will allow for us to understand what takes place within both university and public libraries that impact the daily work of librarians. We hope to gain insight into their experiences outside the typical bounds of library work including experiences with patrons in the library related to mental health, substance use, sexual assault, and criminal behavior. We hope to show a need for more support in these areas if necessary.

 

Will there be any costs related to the research?

None

 

Will there be any compensation for participation?

None.

 

How will your privacy and the confidentiality of your research records be protected?

Survey Monkey may tie your invite information (first name, last name, email) to a response recorded in the system. Identifying information will only be used—if you choose to provide it—for future focus group contact and will be expunged after 1 year. All survey responses will be de-identified once downloaded so that no participant can be personally identified. Only the PIs will have access to the dataset, and the dataset’s existence on the Survey Monkey server will be removed immediately after the data collection phase.
