Are you a survivor of Hurricane Matthew with a disability living in North or South Carolina?

If so, The University of South Carolina needs your input regarding your experiences during the Hurricane Matthew and the subsequent flooding. The information collected will not be identifiable and will be used to enhance Carolina’s emergency response services for persons with disabilities during and after a crisis. The goal of this research is to determine to what extent people with disabilities were impacted by the hurricane. Participation in this study is strictly voluntary and should take approximately 15-20 minutes of your time. 

By completing this survey you are giving us your consent to share your responses with researchers from the University of South Carolina. The data will be used to enhance South Carolina’s emergency response services for persons with disabilities during and after a crisis. No personal identifying information will be gathered unless you volunteer for a follow-up interview.
