Santa Clara Family Health Plan - Book Distribution: Lily the Llama |
Santa Clara Family Health Plan (SCFHP) is donating a new children's book to help kids, and their families understand the importance of vaccinations and why herd immunity will keep us safe. As part of the SCFHP Children Vaccination Readiness Initiative, SCFHP will donate books to local libraries, Family Resource Centers (FRCs), schools, and sites or places where children and families come together to learn.
If you are interested in receiving paperback copies of the book, make your request by completing this brief survey. Once the survey is complete, Jocelyn Ma, Manager of Community Outreach, will contact you within 72 hours.
To qualify for a donation of the book, you must:
- Serve low-resource population groups in Santa Clara County
- Serve children between 3-12 years of age.
- Have a plan to distribute the book this summer or fall (i.e., story time, back-to-school event, resource fair)
- Share one photo of the event featuring the book distribution.
