8.General Purposes Explanation
This form allows parents to consent to or decline School-Based Mental Health (SBMH) services at Immanuel Lutheran School (ILS) in Washington. You may choose to:
- Have your child scheduled for services right away
- Wait until a need arises
- Decline school-based counseling altogether
Even if you do not wish to start services now, completing the form ensures that consent is on file, making it faster to access support later in the school year.
You only need to submit the form once. It is designed to record your preference for each child enrolled at ILS. For example, you may request services for one child but decline or defer for another.
If you decline services for all children, the form will end once that preference is selected.
If you request services now or later for a child, you will be asked for additional information and consent for that child.
