Overview
The event in which you participated was provided by the Nevada State Opioid Response project, which is funded by the Center for Substance Abuse Treatment under the auspices of the Substance Abuse and Mental Health Services Administration.
The intent of this evaluation is to assess the impact of the training on treatment outcomes. Although it is known that well-trained professionals positively impact patient outcomes, the grant needs data to demonstrate that this is true. We believe this evaluation will provide such data.
Evaluation process
You are being asked to complete two brief evaluation surveys. The first survey is the Post-Event Form which is being administered immediately following today’s event. The second is a Follow-up Form that will be sent to you three (3) months from today’s date. The surveys ask you to provide a personal identification code that will be used only for the purpose of matching your answers between the three surveys. Your participation in completing these surveys is completely voluntary and your responses confidential.
