Welcome to the Self-Advocacy Training Provider Post-Assessment!
As we discussed during the Self-Advocacy Provider Training, vendors seeking certification to provide Self-Advocacy Training to STAR Participants and VR Customers are required to pass a post-assessment. On the next page, you will enter your personal and contact information. The assessment begins on the second page and consists of 40 multiple choice and true/false questions. Please read each question or statement thoroughly, and select the answer that is most accurate based on what you learned during Self-Advocacy Provider Training. When you complete the assessment, be sure to click the ‘Submit’ button at the bottom of the page. You will be able to view your score immediately upon completion. In order to PASS the post-assessment, you must receive a score of 80% or higher (32/40).
