Block Grant Survey 2017

Each year we send you a Survey to assess your organization's compliance to the Substance Abuse Block Grant's (SABG) charitable choice requirements. This question is a requirement for all Block Grant funded providers.

This year we are asking additional questions to satisfy other SABG reporting requirements and priorities regarding trauma-informed care, primary healthcare, mental health services, electronic health records and recovery support services.
