Thank you for completing this short survey about how your student travels to and from school. The survey should take about four minutes to complete. Your responses are completely confidential.
Safe Routes to School is a program at your student’s school that helps promote safe transportation habits through education and engineering (new crosswalks, for example). We survey parents and guardians in the fall and spring of each school year as a required part of the Safe Routes to School program. Responses help us make improvements.
This survey should be completed by the parent or guardian of the student. If you have more than one student in your house, please fill this out for your oldest K-8 student. When you've completed the survey, please mail it back to the City of Portland in the envelope provided. Return by November 10th to be entered to win one of four prizes, including rain gear for your student or a $50 gift card!
