You are being invited to participate in a community needs assessment survey for 2019 Community Needs Assessment of Mental Health for the Korean Population in the Greater Toronto Area (GTA). This study is conducted by Hong Fook Mental Health Association, Toronto, Canada. You were selected to participate in this study because you belong to the following ethnic community (Korean). The purpose of this research study is to identify service gaps, in terms of accessing mental health services and inter-professional health care, in the targeted communities. Other objectives of this assessment are to recommend strategies and action plans to minimize the service gaps, to identify practices that maximize impacts and to build or strengthen networks with each community.

If you agree to take part in this study, you will be asked to complete an online survey. This survey will collect information regarding the demographic characteristics of the population, perceptions of mental health and mental illness, and mental health-related needs. The survey will take approximately 20 minutes to complete.

This research may benefit you in several ways. First, your participation in the study may improve your knowledge of mental health and inter-professional care. Second, in the future, you may receive better health care services as a result of this study. 

We believe there are no known risks associated with this research study.

All the survey data collected will be stored under Hong Fook’s Survey Monkey account and will be accessible to only the researchers and Hong Fook staff. All information you supply during the research will be held in confidence. Only aggregated responses will be used in the final report. Individual participants are anonymous and will not be identified under any circumstance.

Your participation in the survey is completely voluntary. You are free to discontinue the participation at any time or refuse to answer any question. If you choose to withdraw from the research, all of your data will be deleted. The decision to withdraw will not affect your relationship with Hong Fook, the researchers or any other group associated with the research.

이 연구는 광역토론토(GTA)에 거주하는 한인을 대상으로 정신건강에 관한 지역사회의 필요성을 조사하는 것입니다. 연구는 캐나다 토론토에 있는 홍푹 정신건강협회에서 주관합니다. 귀하는 한인사회의 일원으로서 이 연구에 초대되었습니다. 연구의 목적은 한인사회 정신건강 서비스 및 관련 전문서비스 이용의 개선 사항을 조사하는 것입니다. 또한, 부족한 부분을 개선할 수 있는 방법과 계획을 건의하고, 효과적인 서비스를 개발하며, 지역사회 간의 네트워크 형성과 강화를 목적으로 합니다.

이 연구에 참여하기로 동의하면, 온라인 설문조사에 응답하게 됩니다. 설문조사에서는 인구 통계적 정보, 정신건강과 정신질환에 대한 인식, 정신건강과 관련한 필요사항에 대한 정보를 수집합니다. 설문조사는 20분가량 소요됩니다. 

이 연구에 참여하면 여러가지 유익한 점이 있습니다. 정신건강 및 관련 전문서비스에 대한 본인의 지식이 향상됩니다. 또한 이번 연구 결과를 통해 필요시 더 나은 정신건강 서비스를 기대할 수 있습니다.

이 연구에 참여하는 데 있어 우려할 요소는 없습니다.  

수집된 모든 자료는 온라인 조사 프로그램인 서베이 몽키(Survey Monkey)의 홍푹 계정에 저장되며, 연구자와 홍푹 직원만 이를 열람할 수 있습니다. 설문조사에서 응답한 모든 정보는 비밀로 관리됩니다. 또한 최종 보고서에는 집계된 자료만 사용되고, 모든 개인은 익명이 보장되며 어떤 경우에도 신원이 드러나지 않습니다. 

설문조사 참여는 전적으로 자발적입니다. 언제든지 참여를 중단할 수 있고, 어떤 질문이든 응답을 거절할 수 있습니다. 만약 도중에 중단하기로 결정하면 입력한 정보는 모두 삭제됩니다. 또한 중단을 한다고 해서 홍푹이나 연구자, 연구 관련자와의 관계에는 아무런 영향을 미치지 않습니다.
