The "Public Health Act 2016" requires local government authorities in WA to develop and implement a Public Health Plan. The plan uses data from literature research and community consultation processes to determine the health issues that are significant within the Town of Port Hedland. The plan outlines how the Town may address these issues through daily operational functions.

This part of the survey aims to achieve the following:
- Identify health issues perceived to be most important to the community
- Identify strategies that may be used by the Town to address health issues
- Determine the perceived quality of health-related services currently provided to the community
- Identify areas of improvement needed around the delivery and accessibility of Town services to benefit community health and wellbeing

The Town of Port Hedland is renewing its Community Safety Plan 2019-2021. 1          The Promoter is the Town of Port Hedland (ABN 19 220 085 226) of Civic Centre, 13 McGregor Street, Port Hedland.

2          By entering this competition, the entrant accepts these terms and conditions.

3          Entry to the competition is free.

4          The competition commences at 00:01am (WST) on 6 February 2019 and closes at 23:59pm on 6 March 2019.  

5          To be eligible, entrants:

(a)        must be over 18;

(b)       cannot be an employee of the Town, or a member of that employee’s immediate family[i]; and

6          Entrants may only enter once and any additional attempts will be disregarded.

7          Entrants agree to have their name published in association with this contest.

8          Prize winners:          

            (a)        will be selected at random on at 11am on 11 March 2019 at the Civic Centre;

(b)       will be contacted on the contact details the entrant provides; and

(c)        must attend the Civic Centre on 9158 9300 to redeem their prize.

9          Prizes:

(a)        the first prize winner will receive a 1x $200 Harvey Norman, Port Hedland Gift Voucher;

(b)       the second prize winner will receive a 1x $200 Retra Vision Gift Voucher, South Hedland;

            (c)        are not negotiable or redeemable for cash; and

(d)       if the Promoter cannot locate a prize winner and/or the prize winner does not collect their prize within 14 days (or as otherwise agreed) of the previous draw, the Promoter will conduct a redraw as soon as practicable in accordance with these terms and conditions.

10        The Promoter’s decision regarding the prize winner is final.

11        The Promoter is not liable for any loss or damage whatsoever, or for any personal injury suffered or sustained, as a result of entering this competition or using any of the prizes.
