* 1. You are invited to participate in a research study examining perceptions of school safety and emergency preparedness in Arkansas public schools. This document provides information about the study. You must be 18 years of age or older to participate. Your participation is voluntary. Please take as much time as you need to read this information, and you may print it for your records if desired.

You will be asked to provide information about your experiences and perceptions related to school safety practices and emergency preparedness. You will also be asked to provide basic background information such as your role in relation to the school. The survey is expected to take approximately 10 to 15 minutes to complete.

There are no anticipated risks associated with participation beyond those encountered in everyday life. You may discontinue participation at any time without penalty. Participation is completely voluntary.

No identifying information will be collected as part of this survey. Your name, email address, or any other personally identifying information will not be linked to your responses. All responses will be reported in aggregate form only. Data will be stored on password-protected devices accessible only to the researcher and will be used solely for research purposes. Anonymous data may be used in future research related to school safety.
