Dear Family Physician / Pediatrician / Allergist,
We are Paediatric Allergists in the Department of Paediatrics at the University of Manitoba and the University of British Columbia, as well as a Family Physician at the University of Manitoba. We are interested in evaluating the beliefs and practices of physicians on the timing of complementary food introduction in order to prevent the occurrence of allergic disease. Your opinions are very important to this study.
We would greatly appreciate it if you would please complete this brief survey. It should take no more than five minutes of your time. The return of the completed survey constitutes assumed consent for taking part in this study. Your confidentiality will be respected. No information that discloses your identity will be released or published without your specific consent to the disclosure. Data from this study will be stored for five years.
