** IMPORTANT: this is **not** a survey about discharge summaries **
The LHD built the Admission/Discharge Notification (ADN) service so that for selected patients you can receive near-instant notification of admission to/discharge from a hospital ward.
Note: ADNs are the short messages the hospital IT systems automatically send to GPs when their Integrated Care patients admit to ward and discharge. ADNs are not sent for ED presentations or for recurring (e.g oncology, renal) admissions and the Discharge notification is *not* the same as a Discharge Summary. They are only activated for certain (e.g. Winter Strategy) patients. Our IT systems usually send them within 5 minutes of an admission or discharge, but the GP's secure message server then needs to download them, so there can sometimes be a longer delay.
