Autism Services Survey 2016

The Centre is committed to continuous quality improvement in all our services.  We are asking you as a person(s) served in the Autism program to complete the following survey.  This survey confidentially comes back to the Executive Director who will then summarize the results.   The questions are based on what we need to know for  our CARF accreditation requirement. Our next accreditation survey will be December 2016.  The results of the survey will be summarized for our annual outcomes reporting done in June of each year.
