This survey has been designed to support you and your club to understand where there are opportunities to increase opportunities for people with disability to be included in every aspect of your club. To gain a broad picture of how inclusive your club us currently, we recommend several people from your club fill out the survey. This could include coaches, players, supporters, club staff and volunteers.

The survey has been designed incorporating work from Simplican et al'.s (2015) model of inclusion and Corazzo, M., & Dyer, J., A New Model for Inclusive Sports? An Evaluation of Participants Experience of Mixed Ability Rugby. Social Inclusion, 2017, Volume 5, Issue 2, Pages 130-140.
