NPD 2021 Covid-19 Hero Award Nomination Application
Nomination Requirements:
Nominator must be a current member of AFP NWA
Respond to the information requested below
Include letters of support for each nominee (maximum of three) – upload in one file
If your nominee is selected, we will request your assistance in obtaining photographs and biographical information for publicity purposes. We will give you the opportunity to notify your nominee of a selected nomination. If your nominee is selected, you and/or your organization are required to engage your nominee in pre-recorded recognition activities and the virtual celebration on November 17, 2021. (Date TBA for the CitiScapes photoshoot and filming of the acceptance speeches.)
Submission Requirements:
Online Nominations must be received by 5:00 p.m. Friday, May 14, 2021.
A maximum of three letters of support may be attached in one file.

The COVID-19 Hero Award recognizes an individual, family, group, organization, business or foundation whose actions and philanthropic contributions have made a significant impact on Northwest Arkansas's crisis response to the COVID-19 pandemic.
Past NPD honorees from any category are eligible for this award.
The nomination should include evidence of:
Financial and philanthropic contributions that made a significant impact on Northwest Arkansas's ability to respond to the COVID-19 pandemic
Support provided to more than one charitable organization in response to the pandemic
Utilization of nominee's sphere of influence to inspire others to support a cause or organization serving our community during the pandemic
Nominees's contribution had a widespread and long-lasting impact in our community
