INTRODUCTION

PHAB has worked with NIHB’s Tribal Public Health Accreditation Advisory Board to develop a Supplemental Process and Documentation Guidance for Tribal Public Health Department Accreditation. This document is expected to serve as a companion document to the PHAB Standards and Measures, Version 1.5  to better assist Tribal Public Health Departments in working through their accreditation journey.


Thank you for providing comments on the proposed Supplemental Process and Documentation Guidance for Tribal Public Health Department Accreditation. We value the opinions of the public health community in ensuring that PHAB’s guidance is clear, relevant, and appropriate.  The vetting period will close December 31, 2017. We plan to incorporate what we learn from this vetting process and will release the final document in early 2018.
INSTRUCTIONS

Please submit your comments to PHAB using this survey instrument. On this survey you can submit your comment and indicate to which Measure your comment pertains. You may comment on as many Measures as you wish. You will also have an opportunity to submit general comments. Do not comment on the format, grammatical errors, or typographical errors. They will be addressed prior to the publication of the final document.

 
PHAB is also interested in collecting any Tribal-specific examples that you may have to accompany the documentation guidance. Any Tribal health department or Tribal public health organization who wishes to submit examples may do so on this form. If you provide examples, PHAB will send you a printed copy of the Standards and Measures, Version 1.5 and a printed copy of the Guide to Public Health Accreditation as well as a packet of other accreditation materials. Please be sure to submit your name and mailing address on the last page of this survey so that we can mail you the packet. One packet per Tribe, please.
