First Foundation Mentor Application Form.

 

This form and any material provided with it will remain confidential and be used by First Foundation for the sole purpose of assessing your suitability for the role of Mentor. If your application is successful this information provided will form part of First Foundation's records of volunteers held by the Chief Executive. Should your application be unsuccessful, your application form and any enclosures will be retained on file for six months and then destroyed. This statement is provided in accordance with the Privacy Act 1993.
