Please complete this survey to submit your Second Year Harvest Season production data following the second application of the approved treatment for your CRAFT Existing Tree Therapies project.
Submit a separate survey for each contract.
CRAFT contract numbers can be found in the top right corner of your participation agreement. If you do not know your contract number, please contact Tina Buice at tina@craftfdn.org.
