Growing in Faith Together: An Adult Faith Formation Program for Risen Savior Catholic Church

Dear Research Participant:

Your participation in a research project is requested. The title of the study is “Growing in Faith Together: An Adult Faith Formation Program for Risen Savior Catholic Church”. The research is being conducted by Sister Annette Lucero, O.P., a student in the Theology and Philosophy department at Barry University, and is seeking information that will be useful in the field of Adult Faith Formation. The aims of the research are to better understand the needs in the area of Adult Faith Formation at Risen Savior Catholic Church. In accordance with these aims, the following online survey through Survey Monkey is being used. We anticipate the number of participants to be 100-150. No information is collected that identifies you in any way.

If you decide to participate in this research, please answer the following survey questions which should take about 10 minutes.

Your consent to be a research participant is strictly voluntary and should you decline to participate or should you choose to drop out at any time during the study, there will be no adverse effects on your membership in the parish.

There are no known risks to you. There are no direct benefits to you however your participation in this study will assist me in completion of my Doctor of Ministry Degree. By completing this survey you consent to participate in this research.
