1. MITS E-Waste Recycling Partnership Application

Thank you for applying for the Rhode Island electronics recycling grant program with Good Point Recycling. This program is funded by Sony, LG, Viewsonic, Acer and other manufacturers seeking to improve collection of used electronics under Rhode Island product stewardship laws. This program will actually pay your facility to collect old TVs and computers. To be considered for this grant program, please provide responses to all of the survey questions below.

* 1. Please provide the name of a contact person at your facility who will be responsible for the management of the used electronics you accept.

* 2. Are you interested in hosting a "One Day Event" (publicized collection day at your facility or a nearby site)? Good Point Recycling offers these as fundraisers (benefits can be to a local school, civic organization, or your own charity). Indicate how many collection events per year you'd like us to sponsor.

* 3. How many sites do you have which could collect TVs, computers and other electronics?

* 4. Which of the following devices are you willing to accept from Rhode Island residents for recycling?

The amount you will be reimbursed will depend upon the number of CPU computer units and TVs per pickup, and the method of delivery (we will pay up to $200 for self-delivery).
