1. INTRODUCTION

* 1. REQUEST FOR REFUND PROCESS: This form is provided by the Madison College Student Senate for students who believe that they should not be required to pay the $46 Commuter Services Fee ($26 for summer), which supports Bus, Para-Transit, Parking & Shuttle services for students. As such, this online form must be completed in its entirety and submitted within the semester (or summer session) for which the student is contesting the $46 fee ($26 for summer). Submissions must be received by midnight of the day on which tuition and fees bills are to be paid in-full. Submission of this form does NOT exempt a student from paying the $46 fee ($26 for summer) by the payment due date written on the billing statement.

Submissions will be considered, decisions made and communicated by mid-October for Fall Semester, mid-March for Spring Semester and mid-July for Summer Session. The decision of the Student Senate will be communicated to the student via the e-mail address submitted by the student. The decision of the Student Senate is final. Refunds will made to the student, as appropriate.

The completion of this form provides information about a student and his or her commuter knowledge, usage, patterns and needs. Form completion time for most students is 5 - 7 minutes.

* 2. EXPLANATION OF SUPPLEMENTAL FEES: Supplemental fees are paid by degree-credit students at Madison-based campuses, including "Senior Audit Students, Contract Students and Dual-Enrollment Students." These students are eligible for Student Activities, Programs and Services. They have access to Fitness Centers, the Health Center and to the Commuter Services Program.

"Youth Option Students, Online-Only Students and Transcripted / Dual-Credit Students" do NOT pay supplemental fees. They are are not eligible for Student Activities, Programs and Services and have no access to Fitness Centers, the Health Center and no access to the Commuter Services Program (bus, para-transit, parking, shuttle), unless they choose a "buy-in option." These students may "buy-into" the Student Activities-Commuter Services-Fitness Health Program by paying $75 per semester or summer session. These students may buy a parking pass ONLY, by paying $25 per semester or summer session.

NOTE: Some categories of non-degree students, by law, cannot be assessed supplemental fees, and therefore if they want access to Student Activities, Programs and Services, Fitness Centers, the Health Center and the Commuter Services Program, they must pay a fee of $75 /semester or summer and specifically request access to the package of activities, programs and services.

Degree-credit students based at regional campuses do NOT pay supplemental fees and are therefore not eligible for Student Activities, Programs and Services. They have no access to Fitness Centers, the Health Center and no access to the Commuter Services Program. (Regional Campuses: Fort Atkinson, Portage, Reedsburg and Watertown.)
