DNR 2026 Recycling Excellence Award Nomination Form

Use this form to submit a nomination for the 2026 DNR Recycling Excellence Awards. Visit the Recycling Excellence Awards webpage for more information.

Nomination guidelines

Nominees who meet the following criteria will be considered:

1. A 2026 nomination form must be submitted on behalf of the nominee including all relevant and supporting documentation before midnight, Sept. 14, 2026.
2. The nominee must comply with the basic requirements of a DNR approved recycling program or Wisconsin state recycling laws.
3. Only ongoing or completed actions will be considered. Nominations based on planned efforts will not be considered.
4. High resolution photos (500 KB and over) are highly encouraged. Please complete and return the photo release form.
5. A nominee can be nominated multiple times for multiple awards but will receive no more than one Recycling Excellence Award every other year.

Before completing this form, make sure you have the following ready:

1. Any written narrative or supporting documents you want to attach, in PDF or Word format.
2. A photo representing the nominee's work.
3. A completed copy of the DNR photo release form.
